The Innovative Models Promoting Access-to-Care Transformation (IMPACT) initiative was a five-year research program providing an opportunity to build upon partnerships, programs, and research to co-create models of care and local interventions that enhance access for vulnerable populations.
This workbook builds on the IMPACT experience to provide an overview of current thinking about implementation, some core principles, and specific checklists for helping groups implement, improve, and sustain locally designed interventions for improving access to care for vulnerable populations.
